
First Visit
Your initial appointment will consist of a thorough comprehensive exam. It will be an appointment for gathering information. Information such as health history, photographs, radiographs (as needed), periodontal screening, and a discussion about what your dental desires are. Please allow a full hour and a half for this appointment. A second appointment will consist of a consultation explaining your diagnosis and treatment options.
Please assist us by providing the following information at the time of your initial visit:
- A completed set of general patient information/dental/medical evaluation forms
- Any current radiographs
- A list of your current medical prescriptions
If you have dental insurance, bring the necessary information. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or guardian must accompany all patients under eighteen to the comprehensive exam appointment.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
Radiographs
If your former dentist has taken radiographs, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our office.
